​​​​​​The 2018-2023 Douglas Cou​nty Community Health Plan sits on a foundation that is committed to creating health equity for everyone in Douglas County. The recent Douglas County Health Equity Report outlines several health disparities existing in our community, including by race, income, education level and geography. A steering committee repr​esenting agencies across the county identified fo​ur issues to target to give everyone in Douglas County a fair and just opportunity to be healthier. With addressing health equity and discrimination as the overarching lenses, the plan focuses on improving four key issues in Douglas County:

​Safe and Affordable​ Housing

  • Increase availability of access to affordable housing, enhance infrastructure supporting collaborative efforts to address affordable housing, increase safety of housing units and expand affordable integrated housing for those experiencing inequity.

​​Behavioral Health

Food Security and a Healthy Built Environment

  • ​​Increase food security and access to healthy foods, including by 2023, increase percentage of low-income residents who can reach a healthy food access point.
  • ​By 2023, increase the percentage of adults p​articipating in the recommended level of physical activity from 22.5% to 25%​

​​Poverty and Jobs

  • ​​Establish collective impact organization from Douglas County organizations that support employment readiness, employment creation and remove barriers that hinder employment.
